Ailments are negative status effects in Titan Quest 2 (TQ2) that can be applied to enemies or players through attacks. They are typically associated with specific Damage Types, stack over time, and their impact grows with the number of stacks inflicted. Ailments bypass resistances and deal True Damage, making them highly valuable in prolonged encounters or damage-over-time builds.

How Ailments Work in TQ2

Ailments are applied in three steps:

1. Attack: The attack must have an Ailment-Chance, which determines whether an ailment is attempted.

2. Define Type: If successful, the ailment type is chosen based on the proportions of damage types in the attack (e.g., an attack that is 70% Fire Damage is more likely to apply Burning).

3. Resist: The target's resistance to ailments is checked. If not resisted, the ailment is applied.

Should Ailment-Chance be above 100%, an additional roll in the “Define Type” step is made to maybe apply a 2nd stack and so on.

Each stack lasts for a limited duration. A single player or creature may apply up to 10 stacks per ailment type, with a combined total limit of 20 stacks from multiple sources. The strength of the effect is defined by the amount of stacks on the target.

Applying an Ailment in TQ2

As an example of how an element is applied: The skill Flame Volley Flame Volley deals primarily Fire Damage. With a sufficient Ailment-Chance, it can apply the Burning ailment. If Flame Volley has over 100% Ailment-Chance, it may apply multiple Burning stacks in one attack, increasing the total damage over time.
